The aim of this course is to provide an understanding of the importance of a multidisciplinary approach in the management of chronic diseases; to develop a holistic care perspective at the individual, family, and community levels; to foster effective communication, collaboration, and coordination among different health disciplines; and to support evidence-based, ethical, and patient-centered care practices. |
