Purpose and content of the course

Course Objectives: The aim of this course is to provide an understanding of the importance of a multidisciplinary approach in the management of chronic diseases; to develop a holistic care perspective at the individual, family, and community levels; to foster effective communication, collaboration, and coordination among different health disciplines; and to support evidence-based, ethical, and patient-centered care practices.
Course Objective: 1.Explain the epidemiology, risk factors, and public health impact of chronic diseases.
2.Evaluate the role and importance of a multidisciplinary approach in chronic disease management.
3.Define the roles and responsibilities of different health disciplines.
4.Develop patient-centered and holistic care plans.
5.Apply evidence-based practices in chronic disease management.
6.Communicate effectively and collaborate within the healthcare team.
7.Plan patient and family education, counseling, and support processes.
8.Support self-care management and lifestyle modifications in chronic diseases.
9.Act in accordance with patient safety, ethical principles, and professional responsibilities.
10.Contribute to quality improvement and continuity of care in healthcare services.

  1) Explains the concept, epidemiology, and characteristics of chronic diseases and analyzes their impact on individuals, families, and healthcare systems.
  2) Describes common chronic diseases (such as diabetes, cardiovascular diseases, chronic respiratory diseases, and cancer) and explains their pathophysiological processes.
  3) Explains the principles of chronic disease management, including prevention, early diagnosis, treatment, rehabilitation, and long-term follow-up.
  4) Describes the roles and responsibilities of different healthcare professionals in a multidisciplinary team involved in chronic disease management.
  5) Explains patient-centered care approaches and their importance in managing chronic conditions.
  6) Describes self-management strategies and patient education principles for individuals with chronic illnesses.
  7) Explains care coordination, case management, and continuity of care in chronic disease management.
  8) Explains quality of life, psychosocial factors, and ethical considerations related to chronic illness care.
  9) Explains prevention and risk reduction strategies for chronic diseases.
